General Information:Carroll County Health and Home Care Services (CCH/HCS) offers seniors and adults living with disabilities, their families, and caregivers the ability to lead happier, healthier, and more independent lives in the comfort and safety of their own homes.Experienced Nurse Case Managers provide in-home assessment and consultation, care planning, care coordination, medication supervision, and ongoing support visits. Licensed Nursing Assistants assist with bathing, transfers, personal hygiene, ambulation, personal care, socialization, and companionship in addition to many other duties. Home Makers and Personal Care Assistants assist with personal care, light housekeeping, including preparation of meals, respite care, companionship, socialization, as well as laundry and food shopping. The services provided by CCH/HCS are very broad and are based on the needs of each individual.
